We are working with organisations in two areas to develop proposals for community health centres. 

The aim of the new health centres is to bring health and social care services together in one building as a form of one-stop-shop for patients. The facilities would promote the co-location of NHS health services with those provided by Local Authorities and the Voluntary & Community Sector. We are supporting organisations to make their business case for funding to NHS England.
